Antenatal screening for Down syndrome and other conditions - Laboratory request form
This form must be used for requesting first trimester screening [MSS1] between 9w-13w6d (best 9w-10w) and second trimester screening [MSS2] between 14w-20w (best 14w-18w).
August 24 2017: Changes have been made to the form (November 2012) to enable it to be completed and signed electronically on-line. In addition, the Current Maternal Weight field and the IVF Pregnancy Yes/No fields are now highlighted in red to draw attention to the importance of ensuring these are completed for accurate risk calculation. The form must still be printed and provided to the woman.

Genetic Services Referral form
Genetic services can provide information and support for families with, or at risk of, a genetic disorder. Referrals to Genetic Services must come from the LMC in the first instance.
