Screening histories should be sourced from Regional Services sites, as usual.

Please note that  new colposcopy forms should be used. Copies were mailed to colposcopy services during the week ending 27 June 2007. They can also be ordered direct from the Register Central Team. Copies of the forms are available below.

New Zealand Health Network

National Health Index (for digital certificate)

For those wishing to access screening histories via a web page, this option is available in the new register.

In addition to acquiring access to the Health Network and a digital security certificate, a completed User Access Form is required (available below).

Training on the web user interface is now available for those organisations that have Health Network access, a valid digital security certificate and can open the register logon screen from a web browser.
