This page contains the forms required for requesting information from the National Screening Programmes’ data sets.

The definition of information is

raw data that has;

• been verified to be accurate and timely,
• is specific and organised for a purpose,
• is presented within a context that gives it meaning and relevance, and which
• leads to increase in understanding. This can be in the form of a report, document, spreadsheet or a graph.

Information is released according to the following:

Health Act 1956

Health (Cervical Screening (Kaitiaki) Regulations 1995)

All requests must be accompanied by the appropriate request form (NCSP or BSA) and should be sent to:

Please note that for all requests from the NCSP Register where Maori data is requested an application for NKG approval must accompany the request (this is protected information under the Health (Cervical Screening (Kaitiaki) Regulations 1995.
